The purpose of the Registration Form is to provide basic information about the patient including contact and insurance information. One will be provided at our first meeting or you can save time by printing it here and bringing in a completed form.

A signed Release of Information provides permission for professionals to exchange information about the patient. It should be completed by the patient or in the case of minors, by the parent or legal guardian. The form is completely voluntary in that patients may choose to keep all information confidential. It can be printed here for convenience or completed during office visits.
