Before Your First Visit.

You might have to perform a few easy tasks before your first visit. You should contact your insurance company to learn about your coverage as it relates to counseling/psychotherapy. This includes the amount you will pay for copays, information regarding deductibles, and whether prior authorization is required. Insurance policies change frequently, so this step is strongly advised. Many insurance policies now have deductibles. Essentially, the insurance company will not pay for services until you have paid a set amount to providers.
Please fill in the information below and click SUBMIT, in order to provide me with your full name, date of birth, address and contact information. Please include the name and phone number of your insurance company, as well as your insurance I.D. number and authorization number if one was required.

When You Arrive for Your First Visit
Your first visit is an opportunity for us to determine how counseling may be helpful to you. We will discuss your current situation and history as they relate to the purpose for your visit. We will, at your pace and comfort level, explore challenges you may be facing at work, school, or in interpersonal relationships.
I will answer any questions you may have relative to symptoms you may be experiencing, and provide you with a realistic assessment of what you can expect from treatment. We will likely discuss various treatment options and together determine what course of treatment is best for you.
We will also talk about the frequency of visits, the expected duration of treatment, and the office policies regarding fees for copays, deductibles and missed sessions. |
